A 42-year old female cocaine abuser is brought to the ED complaining of “pressing” chest pain in the left sternal border. The pain started half an hour ago after using cocaine. She is diaphoretic and complains of pain radiating to both her arms. Her vitals are BP 175/120, HR 120, RR 30, and O2 sat of 99% in room air. The ECG reveals extensive anteroseptal ST elevation with reciprocal ST depression in the inferior leads. Transferring the patient to a cath lab is expected to take between 90 and 120 minutes.
