A 24 year old male of 70 kg weight took a large dose of acetaminophen in an intention to commit suicide. He took 80 tablets of 500 mg tablets of acetaminophen in a one time dose. He was brought to the emergency department 3 hours post ingestion, and had an altered level of consciousness. His initial blood work drawn at presentation shows an acetaminophen level of 6000 umol/L, and a lactate of 9. How would you manage this patient?
